Service Summary
The Huron County Community Care Access Centre (CCAC) provides home care services to individuals in their home to shorten hospital length of stay or delay, and possibly prevent, institutional admission. Professional services include nursing, occupational therapy, physiotherapy and speech pathology. Social Work and nutritional counselling can also be accessed.
There is a well established link with the Community Care Access Centre. Case managers attend multidisciplinary conferences and see patients in the Hospital. The acute care component of the CCAC provides a comprehensive range of services including physiotherapy, occupational therapy, speech/language pathology, nursing, social work, and nutritional counseling. Respiratory therapy services are provided by physiotherapists in consultation with staff from a private company that supplies oxygen equipment. Complex care is provided as part of the home care program, including limited intravenous therapy and tracheostomy care.
CCAC works in the AMGH hospital to provide discharge planning.
