All Employees (Student Orientation below)
Thank you for choosing Alexandra Marine and General Hospital as your employer! We are looking forward to having you join our team. All of our employees are valued for their role in achieving excellence in patient care.
Within 3 days
Complete the following and send to Human Resources within three (3) business days by email amgh.humanresources@amgh.ca, fax 519-524-8537 or in person; failure to do so will render the Letter of Offer null and void. Once you have done so, please make arrangements to have an initial meeting with Human Resources (519-524-8689 Ext 5720) and Occupational Health and Infection Control (519-524-8689 Ext 5721) prior to your orientation for the following (failure to provide these documents will result in the inability to attend orientation and being scheduled shifts)
Before your Orientation you will have an Initial Meeting with Human Resources
Complete the ELearning for New employees prior to your meeting. Log in information will be provided to you by the human resources officer.
Please bring with you to your meeting:
- SIN & valid picture identification
- Valid registration/license if applicable which will be copied to your personnel file
- Void cheque for payroll
- Completed TD1 Tax Forms
- Hospitals of Ontario Pension Plan (HOOPP) Waiver form only if you wish to opt out of the pension plan
- You will have your picture taken at this meeting for your AMGH photo ID.
Before your Orientation you will have an Initial Meeting with Occupational Health
Please bring:
- Completed Medical Forms
- Proof of immunity to Hepatitis B, MMR, Chicken pox and Tetanus, diphtheria and Pertussis.
- Proof of a negative TB skin test in the last 12 months
Before Orientation
Follow the mandatory steps below required prior to your start date, to prepare for your employment at AMGH.
Complete the eLearning for New employees prior to your Orientation-insert link. Instructions on how to access will be sent to you via email with your login information.
Vulnerable Sector Check-Your offer of employment is conditional upon the satisfactory completion of a Vulnerable/Criminal Sector Check. If you have a Vulnerable/Criminal Sector check not older than 12 months we will accept it; if not, please take the attached Request Letter and
form to your nearest OPP office (you can also visit http://www.opp.ca/ecms/index.php?id=499 to use the online form to enter your data and print). Once you have received your completed check, please bring it to Human Resources for verification and it will be copied to your personnel file; the original copy is your copy to keep. Photocopied checks will not be accepted.
If you require assistance completing any of these tasks because of a disability, the Human Resources officer will make sure you are able to complete it on your first day. Please notify the Human Resources before your orientation of a disability.
Orientation
An in-person orientation session will take place on your first day. Once orientation is complete, you are officially ready to begin working at AMGH.
Your department will provide any necessary training that is specific to your role.
AMGH has a culture of continuous learning for our staff, and a focus on quality improvement for both our staff and patients. Over the course of your employment at AMGH, you can expect ongoing assessments and performance reviews.

Orientation for Students
All Students
Before your start date
We are happy to have you join us for your clinical placement. You will attend orientation on your first day, prior to your clinical hours. Prior
to your placement we require the following documentation, which must be returned to me two weeks prior to your orientation date (failure to provide
these documents will result in the inability to attend orientation and being scheduled clinical shifts):
- Complete the ELearning for New employees prior to your Orientation-insert link
- Medical forms completed
- Proof of a negative TB skin test in the last 12 months.
- Proof of immunity to Hepatitis B, MMR, Chicken pox and Tetanus, diphtheria and Pertussis.
- Vulnerable Sector Check no more than 12 months old (photocopies will not be accepted; we will take a copy and return the original to you)
Before Orientation
- Human Resources will notify you with your date for General Orientation.
- Complete the ELearning for New employees prior to your Orientation and complete the Elearning Series for Manadatory Elearning. Instructions on how to access will be sent to you via email with your login information.
Orientation
An in-person orientation session will take place on your first day. Once orientation is complete, you are officially ready to begin working at AMGH. We will have your preceptor, contact you regarding shifts for your clinical placement.
